Required: Cart page fields

Great app and even greater and really fast support! That's how it should be! As far as I know the only app which can help you to get additional fields into the cart without having the shopify plus plan. 100% Reccomendation!

Does what it says, does it well and inexpensively, to boot! The dev has been super helpful and responsive in guiding me through integrating the fields into pages in my theme!

This App is a small but important part of our operation and it works well. Just as important, the developer's customer service is outstanding!

Great little user friendly app for people who can't/don't want to code the questions in themselves by hand. Fills a great little niche and support was excellent, they even made a small revision to the app on my behalf to improve it. 10/10
